Patients aged b18 yr, who had undergone routine total primary replacement of one knee, performed under a standardized regimen of spinal anaesthesia, were included in the study. All patients received a single dose of spinal bupivacaine 1020 mg, and intraoperative sedation with midazolam 0.51.0 mg i.v., or propofol 6 mg kg1 h1. Eligible patients were in satisfactory health, as determined by medical history and physical examination. Women were eligible for the study only if they were not lactating and veried as not being pregnant. Patients were excluded from the study if they had undergone a revision to a previous knee replacement, emergency knee replacement, or knee replacement as a result of a trauma. Patients with asthma or bronchospasm, requiring treatment with glucocorticoids, were excluded, as were those with inammatory bowel disease, a chronic or acute renal or hepatic disorder, or a signicant coagulation defect. Alefacept is a chimeric protein combining CD58 immunoglobulin-like domain 1 with human IgG1 Fc. Alefacept mediates adhesion by bridging CD2 on T cells to activating Fc receptors on effector cells, but the equilibrium binding parameters have not been determined. Alefacept mediated T cell killing by NK cells and adhesion between CD2- and CD16-expressing cells at an optimum concentration of 100 nM. We introduce novel measurements with supported planer bilayers, from which key twodimensional and three-dimensional parameters can be determined by data fitting. Alefacept competitively inhibited cell bilayer adhesion mediated by the CD2CD58 interaction. Alefacept mediated maximal adhesion of CD2 T cells to CD16B, an Fc receptor, in planar bilayers at 500 nM. A mechanistic model for alefacept-mediated cell-bilayer adhesion allowed fitting of the data and determination of two-dimensional binding parameters. These included the density of bonds in the adhesion area, which grew to maintain a consistent average bond density of 200 molecules m2 and two-dimensional association constants of 3.1 and 630 m2 for bivalently and monovalently bound forms of alefacept, respectively. The maximum number of CD16 bound and the fit value of 4, 350 CD2 per cell are much lower than the 40, 000 CD2 per cell measured with anti-CD2 Fab. These results suggest that additional information is needed to correctly predict Alefacept-mediated bridge formation and allergen.

It is speculative whether the clonality data represent a ; an acquired clonal hematopoietic disorder with a transient polycythemic phenotype, b ; "pseudoclonality", 44 or c ; "somatic selection after X inactivation" such as seen in female carriers of some X-linked congenital disorders.45, 46 Nineteen patients with congenital or familial polycythemic or thrombocythemic disorder had all polyclonal hematopoiesis. Of 17 subjects with congenital or familial polycythemia, 7 had elevated or inappropriately normal Epo level for their Hct, 5 7 were analyzed for mutation of von Hippel Lindau VHL ; gene; 2 5 had VHL mutations.47 Remaining 8 subjects had low Epo level, hypersensitive erythroid progenitors to Epo and most had familial history fulfilling the criteria for the diagnosis of PFCP.43 Interestingly, some of these polycythemic subjects with polyclonal hematopoiesis were previously diagnosed as PV while they had transient presumably reactive ; thrombocytosis or mild splenomegaly, and other minor laboratory criteria of clinical diagnosis of PV.30.

The evolutionary relationships of these strains and some others involved in the analysis are shown in figure 1. This cladogram agrees with phylogenetic analyses of the rrn16 and tufA genes figs. 2 and 3 ; . Relative-rate tests showed increased substitution rates in rrn16 and tufA, compared with green relatives, in the two P. uvella species. However, sequence analyses showed that the genes were subject to selection and therefore functional. The increase in substitution rate was greater at sites subject to less stringent selection, implicating a partial relaxation of selection. The tufA gene of P. obtusum showed a large reduction in codon preference, suggesting that the relaxed selection is due at least partly to a reduced load of protein synthesis. This was proposed earlier for the increased substitution rates in Epifagus Wolfe et al. 1992 ; , but alternative explanations were not ruled out. We observed no increase in the substitution rate in the branch leading to P. oviforme, suggesting that photosynthesis was lost more recently in this lineage. This is the first analysis of the molecular evolutionary consequences of the loss of photosynthesis in a chlorophyte alga.
