Etidronate generally was very well tolerated. Only two subjects in the placebo group ; discontinued the study because of AEs during the first 24 months of study. In addition one patient former placebo ; withdrew because of AE during the follow-up year. Table 3 shows the AEs most frequently reported in the first 2 yr of study. The majority of AEs reported were mild in severity and the incidence was comparable between groups. No severe gastrointestinal AEs were reported. Mild abdominal pain occurred in five subjects one in the placebo group, and four in the etidronate group ; . All five subjects had a prestudy history of gastro-intestinal problems. The investigator reported that each of these five subjects experienced intolerance to the calcium supplement. Serious AEs were relatively more frequent in the placebo group. In the follow-up year two subjects in the former placebo group were hospitalized. Vertebral and nonvertebral fractures. One subject in the etidronate group sustained a traumatic fracture of the first lumbar vertebra after lifting a 60 kg man. The investigator reported that the fracture, although responsible for a severe pain, was doubtfully related to the study drug. The subject continued in the study. Three subjects in the placebo group and two in etidronate experienced traumatic nonvertebral fractures and exenatide.

Aspergillus fumigatus is a severe fungal pathogen of immunocompromised patients commonly treated with antifungal agents such as amphotericin B. However, in many cases, treatment is ineffective and infection often leads to death. Glutathione S-transferase GST ; genes identified in Schizosaccharomyces pombe and A. nidulans have indicated a role for GST in mediating anti-fungal drug resistance. However, the role of GST in A. fumigatus has yet to be elucidated. GSTs belong to a family of multifunctional enzymes that are involved in cellular detoxification of a broad range of xenobiotics. GSTs function by catalysing the conjugation of reduced glutathione GSH ; with the target xenobiotic, which aids in removal from the cell. GSTs have been divided into a number of subclasses, including alpha, mu, pi and theta, based on sequence similarity, substrate specificity and immunological cross-reactivity. Homology searching of the A. fumigatus genome with S. pombe GST sequences identified the presence of two putative GST open reading frames, namely GST1 and GST2. A third open reading frame was identified by interrogation of the database with an A. nidulans GST sequence GST3 ; . GST1 and GST2 open reading frames of 663 and 675 bp respectively were amplified by PCR and cloned into the protein expression plasmid pProEx-HtaTM. His-tagged recombinant proteins were produced in E. coli, which were then purified using the affinity tag. Mass spectrometry was utilised to confirm the identity of rGST1, and similar work is ongoing with rGST2. The activity of GST enzymes against various substrates is a key factor in the identification of GST class, and both rGST1 and rGST2 were shown to have low level activity against the "universal" GST substrate CDNB. Activity may be diminished due to the additional His-tag present, but low activity is also an indicator of theta class GSTs. No activity was detected with rGST1 or rGST2 for the additional substrates DCNB and ethacrynic acid. One characteristic of theta class GSTs is peroxidase activity, and rGST1 was shown to be active against cumene peroxidase. Assays are ongoing with rGST2. Examination of sequence data reveals similarity of GST1 and GST2 at conserved sites common to theta class GSTs, indicating that A. fumigatus GST1 and GST2 may also be theta class. Future analysis includes antisera generation, induction of native GST production with xenobiotics and examination of native expression levels.
