Bone pain. Patients in the ibandronate 6 mg group showed a significantly improved bone pain score over time compared with the placebo and ibandronate 2 mg groups Figure 3 ; . Furthermore, patients treated with placebo and the 2 mg dose experienced a transient decrease, then an increase, in pain score. The 6 mg patient group experienced a rapid initial decrease in pain score that remained below baseline score throughout the study. Analgesic use was lower in both ibandronate groups than in the placebo group, indicating that the improvement in pain was not due to increasing use of analgesics [20].

Possible side effects of ibandronate : all medicines may cause side effects, but many people have no, or minor, side effects. Dose-related improvement in patient-reported symptoms compared with placebo [34]. However, at a dosage that provided efficacy less than that of ipratropium, the incidence of dose-related, mechanism-based side effects for OrM3 exceeded those observed for ipratropium [34]. Bronchodilators are still central in the symptomatic management of COPD [1]. For this reason, the current opinion is that it will be advantageous to develop inhalers containing combination of several classes of long-acting bronchodilator drugs in an attempt to simplify treatment regimes as much as possible. Consequently, several options for once-daily dualaction ultra LABA + LAMA combination products are currently being evaluated Table 3 ; . Because combination therapy with an ICS and a LABA is now considered a therapeutic. In a further syngeneic mouse model pamidronate in continuous injection was showed to inhibit the intraperitoneal growth of murine myeloma cells and to reduce the tumor weight by more than 50 % compared with the control MULLER et al., 1996 ; . In contrast to these results, Kostenuik and coworkers found that skeletal tumor burden was increased after pamidronate treatment of rats with metastatic Walker 256 cancer cell tumors KOSTENUIK et al., 1993 ; . In another study of Shipman, the potent bisphosphonate ibandronate failed to induce myeloma cell apoptosis or to reduce tumor burden in the 5T2MM murine model of multiple myeloma. SHIPMAN et al., 2000 ; . These results underline the fact that dose selection, tumor type and other factors need to be carefully assessed for each bisphosphonate before entering clinical trials. Furthermore, at the doses and regimens used so far in animal models, the antitumoral effect of bisphosphonates seems to be limited to bone, with little impact on the nonosseous metastases. This could indicate that the interference of bisphosphonates in the communication between osteoclasts, bone- and malignant cells, is responsible in fact for the beneficial local effect of bisphosphonates. It is also possible that the high local concentration of bisphosphonates in bone play an important role in activity of these drugs on bone tumor burden. The above hypothesis was verified by treating HepG2 cells or primary hepatocytes with atorvastatin in the absence or presence of mevalonate Figure 3 ; . Mevalonate was expected to prevent the cholesterol depletion caused by statin because it is the product of the reaction catalyzed by HMG-CoA reductase. Analysis of NARC-1 and LDLR expression levels showed that addition of mevalonate efficiently reversed NARC-1 and LDLR upregulations. As in Figure 2, NARC-1 upregulation was higher than that of LDLR 2-fold ; . Interestingly, NARC-1 downregulation by addition of mevalonate was also more drastic than that of LDLR. It is important to note that in HepG2 cells and hepatocytes at 1 mol L statin, mevalonate lowers NARC-1 but not LDLR ; level below that of the control, suggesting that PCSK9 is regulated more tightly by cholesterol than the LDLR gene.
