Sarah White, Elisabeth Hill, Uta Frith, Institute of Cognitive Neuroscience, University College London Background: The performance of individuals with Autism Spectrum Disorder ASD ; across intelligence subtests is characterised by an uneven profile. Objectives: To relate performance on cognitive tests, generated by three major theories of the core symptoms of ASD, to standardised intelligence subtests. Methods: 57 high-functioning children 6-12 years ; with ASD and 27 controls were tested on the WISC, as well as a battery of tests tapping theory of mind ToM ; , central coherence CC ; and executive function EF ; abilities. Results: Children with ASD who had a personal peak on the block design subtest showed elevated performance on a test of weak CC compared to the remaining children with ASD. Performance on these two tests was also correlated across the whole ASD group, as well as within the control group. In line with previous research, ASD children with a personal dip on the comprehension subtest had weaker ToM performance than the remaining ASD children, and these tests were correlated across the whole ASD group but not in the controls. EF performance was not related to one particular subtest but more generally to verbal intelligence in the children with ASD, whilst correlating with the object assembly subtest in the controls. Conclusion: ToM impairment and weak CC both contribute to the uneven profile characterising ASD intelligence test performance. EF impairment appears to have a wider effect across verbal intelligence. These same relationships were only seen in the control children for weak CC, indicating a cognitive style rather than a deficit in this domain. Surprisingly, no scientific investigation of this phenomenon has been conducted to date. Objectives: Discovering the basis of exceptional autobiographical memory. Methods: Standardized and unstandardized tests of memory functions were administered to a 21 year-old male with Autism displaying outstanding calendar calculation skills. Results: The patient was largely impaired on standardized tests of episodic and semantic memory. Nevertheless, he had an exceptional memory for events of his own life. For instance, he could accurately remember the weather for the last 7 years, and he was able to recall virtually all the dates when he met with the researchers over a period of 3 years. Furthermore, he seemed able to recall detailed contextual information for all these dates. In a remember-know paradigm he claimed to remember meeting with the researcher in about 90% of the cases. He was also able to learn lists of non-meaningful future dates with a level of performance well beyond the performance of undergraduate students. However, in this task he reported knowing rather than remembering in most of the cases. Conclusion: The results of this study suggest that autobiographical memories are actually retrieved from the medial temporal lobe episodic memory system rather than from a semantic memory store as factual information would be ; . The present investigation focused on bite force in relation to a ; Angle's classification, b ; craniofacial dimensions, c ; occlusion, and d ; head posture. It was the aim of the study to analyse which parameters were most important for bite force when tested for the effect of age. Such an analysis has not been previously performed. Subjects The sample comprised 88 children 48 girls, 40 boys ; aged 713 years Table 1 ; , sequentially admitted for orthodontic treatment at three municipal dental health services in North Zealand, Denmark. The children were selected by the Danish procedure for screening the child population for malocclusions entailing health risks Danish Ministry of Health, 1990; Solow, 1995 ; by the orthodontic specialist in charge of the clinic concerned. Most of the children were Caucasian. There were no gender differences in the distribution of the stage of dental eruption and age Sonnesen et al., 1998 ; . None of the children had craniofacial anomalies, obstruction of the upper airway or systemic muscle or joint disorders and only 7 per cent of the children were evaluated as having a need for referral for TMD treatment Sonnesen et al., 1998 ; . A 69-year-old woman with hypothyroidism, hypertension, and previous multiple strokes, and who had undergone a partial gastrectomy, had been receiving daily doses of ferrous sulfate. She remembered aspirating an iron pill in October 1999 when she experienced choking, wheezing, and cough. The findings of a chest radiograph were normal. Two months later, she was hospitalized with pneumonia and required assisted ventilation. A bronchoscopy at that time revealed extensive inflammation of the truncus and left lower lobe bronchus LLLB ; . No FB was detected, but the truncus was lined with a greenish brown necrotic material Fig 1 ; . As she remained symptomatic, a repeat bronchoscopy 3 months later revealed near-total obstruction of the truncus and LLLB with purulent secretions from the left lower lobe. The respiratory cultures were negative for mycobacteria and fungi but were positive for methicillin-resistant Staphylococcus aureus. The patient was treated with IV vancomycin, and a BB was attempted without success. She was referred to our institution for further management. The truncus and LLLB were found to be 90% and 70% stenosed, respectively, the etiology of which was unclear at that time. Near-total patency was established by BB, and endobronchial injection of methylprednisolone was carried out. The patient began receiving therapy with oral steroids and antibiotics after the procedure. Despite treatment, she developed recurrent pneumonia from restenosis of the affected bronchi. Endobronchial biopsies of the truncus 1 year after the episode of aspiration revealed ferric iron. Daytime wetting or diurnal enuresis is more common in girls. A thorough medical evaluation is always indicated to identify the cause of the daytime wetting. Two types of daytime wetting amenable to biofeedback will be discussed here. These include "dysfunctional voiding" and "bladder instability". Dysfunctional voiding also known as Hinman's Syndrome or "non-neurogenic neurogenic bladder" ; occurs when there is dyscoordination in voiding between the bladder smooth muscle contraction and opening of the external urethral sphincter in the absence of neurologic dysfunction or disease. Children with the disorder typically have urinary urgency, wet themselves in the day or night, have increased or decreased frequency, and may sustain recurrent urinary tract infections. Children with dysfunctional voiding may squat, jump, or posture in an effort to contain urine in the face of bladder contractions against a closed urethral sphincter. Constipation and stool soiling encopresis ; are sometimes a part of the picture and children have a difficult time relaxing the urinary sphincter as they struggle to keep their anal sphincter contracted to withhold stool. Therefore, treatment of any underlying constipation is often essential. The disorder is annoying at least, but also can result in bladder changes and disorders of the ureters and kidneys. The bladder contractions against a closed urethral sphincter cause thickening of the bladder wall, and a diminished bladder capacity. This increases the force of the contractions and leads to a vicious cycle of contracting and resisting more and more. In addition to the changes in the bladder wall, there may be ureteral reflux or kidney sequelae as well. Biofeedback may be necessary to teach a child the feeling of relaxing the urinary sphincter at will, in response to bladder contraction while on the toilet. Other treatments include frequent voiding regimens and anticholinergic medications to relax the bladder. More recently there have been descriptions and reports of bladder instability treated with urodynamic biofeedback.
