The seeds of the milk thistle plant are commonly used to protect the liver from damage caused by hepatitis viruses as well as alcohol and other substances. Compounds found in milk thistle -- sylibin, sylimarin -- act as antioxidants and also stimulate the repair of the liver. But now it appears that these and possibly other compounds in milk thistle can have other effects. Researchers at the University of Pittsburgh have suspected that milk thistle can slow down or reduce the activity of enzymes in the liver. What does this have to do with HIV? you might ask. Well, enzymes in the liver break down many of the substances that we eat and drink, including medications. If the activity of these enzymes are reduced, then drugs remain in the blood longer than they otherwise might. This could lead to having higher-than-expected levels of drugs in the body, causing side effects or intensifying already-existing side effects. Indeed, in recent experiments using milk thistle and human liver cells, the researchers found that relatively small concentrations of milk thistle did significantly slow down the activity of the liver enzyme CYP3A4 by 50% to 100%. Many medications taken by people with HIV AIDS PHAs ; -- such as protease inhibitors and non-nukes -- are processed by this liver enzyme. If milk thistle is taken by someone using protease inhibitors or non-nukes, it has the potential to raise levels of these drugs, causing unpleasant or even dangerous side effects. Nonlanguage Cognitive Deficits and Hallucinations in Schizophrenia TO THE EDITOR: Ralph E. Hoffman, M.D., and colleagues 1 ; reported impaired performance of hallucinating schizophrenic patients on a narrative speech perception task and a sentence-repetition task relative to nonhallucinating patients and normal comparison subjects. However, the hallucinating and nonhallucinating patients did not differ on a measure of continuous performance, which indicated that the differences in speech perception were not due to deficits in sustained attention. Dr. Hoffman et al. concluded, "Results support the hypothesis that hallucinated voices in schizophrenia arise from disrupted speech perception and verbal working memory systems rather than from nonlanguage cognitive or attentional deficits" p. 393 ; . We think that the first part of the conclusion is adequate i.e., it is supported by the data ; , but the latter part may be premature. Specifically, the two nonlanguage cognitive functions that have figured most prominently in the cognitive theories of hallucination-- namely, reality monitoring and imagery vividness--were not assessed by the authors. Previous studies have revealed differences between hallucinating and nonhallucinating patients on tasks measuring these functions e.g., references 24 ; . Although, as Dr. Hoffman et al. pointed out, one PET study of hallucinations failed to find an activation of Broca's area, which challenges the reality-monitoring account that hallucinations are misidentified thoughts. All studies of functional brain imaging during auditory hallucinations e.g., reference 5 ; have demonstrated activation of the temporal areas, consistent with speech perception as imagery accounts auditory imagery has been shown to activate temporal auditory association areas [6] ; . In addition, the speech perception tasks used by Dr. Hoffman et al. may involve mental imagery to a significant extent. The process of filling in the blanks during speech perception to reduce acoustically ambiguous speech is clearly dependent on imagery. Indeed, Kosslyn and Sussman 7 ; have argued that "immaculate perception" does not exist: in most circumstances, perception and imagery are intimately intertwined. This underscores the importance of a thorough investigation of the differences between patients with and without hallucinations on multiple tasks aimed at measuring language-specific e.g., speech perception ; as well as non-language-specific e.g., reality monitoring and mental imagery ; cognitive functions that may be involved in the cognitive basis of hallucinations. It is too early yet to discard the possible contribution of disruptions in nonlanguage cognitive functions to the occurrence of hallucinations in schizophrenia and secobarbital.

The fact that systemic blood levels of histamine were not elevated during NT infusion argues that the histamine involved in NT-induced TC uptake was produced locally. These changes have been attributed to the stabilizing effects of NO on intestinal mast cells 35, 53 ; . Here we found that basal and NT-induced TC uptake were inhibited in animals receiving NO donor L-arginine, whereas they were enhanced in animals that received NOS inhibitor L-NAME. These data support our contention that the enhancement of intestinal TC uptake seen in our experiments is caused by mast cell-mediated increases in intestinal permeability, and they also illustrate that this process can be modulated by NO. It is possible that NT altered the luminal [3H]TC during our experiments; however, this would not provide an explanation for the response observed, because NT stimulates secretion of fluid and mucus 21 ; that would likely act to diminish the rate of [3H]TC uptake. Whereas active transport of TC occurs in ileum, the primary mechanism operating in jejunum is likely to be carrier-mediated transport 41 ; . NT has little effect on TC uptake in ileum, but as shown here it markedly enhances TC uptake in proximal intestine 25 ; . Because NT also enhances intestinal uptake of substrates absorbed primarily via the paracellular route [3H]mannitol, [51Cr]EDTA ; 25 ; , it is conceivable that some of its effect on TC uptake involves increased paracellular permeability. Considerable evidence links mast cell activation and mast cell mediators 5, 54 ; to increases in paracellular permeability observed during development 14 ; and in pathological conditions involving stress 62 ; , food allergy 39 ; , enteritis 12 ; , and ischemia 61 ; . On the other hand, some findings are at odds with the hypothesis that NT enhances TC uptake by increasing intestinal paracellular permeability. In the rat, NT secretion 51 ; and BA uptake 31 ; are enhanced in the fed state, yet intestinal paracellular permeability is diminished compared with the fasted condition 63 ; . Although it is possible that NT stimulates TC transport at the level of the epithelium, it seems more likely that its effects on vascular permeability enhance the passage of TC into the circulation. It is commonly thought that passage across the intestinal epithelium is rate determining for absorption of conjugated BA 29 however, the dependence on vascular permeability has not been thoroughly examined 29 ; . Because NT and mast cell mediators alter blood flow and dramatically increase vascular permeability to albumin, it seems likely that these effects contribute to the enhancement of TC uptake in the intestine. In conclusion, basal and NT-stimulated TC uptake in the rat intestine are similarly dependent on mast cells, are largely mediated by release of mast cell mediators, and are subject to regulation by NO. These findings are consistent with the idea that intestinal NT is an endogenous regulator of EHC whose effects are mediated by the activation of intestinal mast cells and sandimmune.
