Expecting another surge in human infections in Northeast Asia as the winter months approach. There are dozens of different known flu strains, named for two particular proteins that each virus carries. H5N1 refers to the bird flu strain that emerged in Hong Kong in 1997, killing or forcing the destruction of 1.5 million chickens, ducks and geese, infecting 18 people and killing six. Migratory birds, usually wild ducks, are the natural "reservoir" of bird flu viruses, and usually do not become sick when infected. Domestic poultry, including chickens and turkeys, die quickly when infected. Distribution The H5N1 strain first emerged in Hong Kong in 1997, where it killed six people, and surfaced again on the Korean peninsula in 2003. It has since been found in birds in Cambodia, China, Indonesia, Japan, Laos, Malaysia, Mongolia, Philippines, Taiwan, Thailand and Vietnam. The disease has also struck six Russian regions and Kazakhstan, causing the death of nearly 14, 000 fowl. Migratory birds are expected to spread the disease further into the former Soviet states in the coming months. The outbreak was discovered in mid-July in Novosibirsk and has spread through Tyumen, Omsk, Kurgan, Altai and now Chelyabinsk, which is about 1, 000km 625 miles ; from Novosibirsk. Vaccines Ordinary flu vaccines do not provide any protection against bird flu. Several companies are working on a H5N1 vaccine. Australian company CSL Ltd is at an advanced stage of research and has just begun human trials. The Australian government gave CSL A million in July to fast-track development of a vaccine. France's Sanofi-Aventis has developed another promising vaccine. This has proved effective at stimulating an immune system response to bird flu in healthy adults. However, the doses of antigen required are very large. US-based Chiron Corp. aims to test its vaccine later this year and Britain's GlaxoSmithKline Plc plans large-scale clinical trials in 2006. However, these may not be effective should a pandemic break out. This is because researchers cannot forecast what a pandemic-capable virus would look like, so they cannot be sure that any vaccine developed now will actually work. Anti-viral Drugs Though not specifically designed to combat the H5N1 virus, two antiviral drugs can help fight the infection and may even prevent it, if taken regularly. These are Roche's Tamiflu, known generically as oseltamivir, and GlaxoSmithKline's Relenza, or zanamivir. Of the two, Tamiflu is regarded as the safest and most effective. About 30 countries are now stockpiling this drug. More recent reports suggest that strains of the virus in Japan and Vietnam are becoming resistant to Tamiflu. This highlights the continuing need for a vaccine to be developed.

Editor--The POEM headline "Half of all ganglions resolve spontaneously" is likely to lead to dangerous conclusions about a common hand condition among casual readers.1 The text discusses a form of ganglion that constitutes only 18-20% of all such lesions, which is not made clear in the bottom line. This POEM draws on the evidence of a flawed paper and then suggests its findings apply to all ganglia. Patients do not like these lesions, and the commonest dorsal form 6070% of cases ; causes discomfort. In my experience, symptoms are closely related to the degree of inflation of the lesion, waxing and waning in unison, which suggests they are probably caused by local compressive effects. Patients come to hand specialists seeking removal, and the quoted article by Dias and Buch makes it quite clear that the selection of patients to be managed by masterly neglect was not impartial or objective: "the surgeon's preference dictated" whether surgery or observation was chosen.2 The case made for non-operative management is further weakened by the manner of arrival at figures for post-surgical recurrence, said by Dias and Buch to have been 42%.2 Volar wrist ganglion surgery is challenging and yet was carried out by a mixed bag of "senior and junior surgeons." In properly experienced hands, however, the recurrence rate after surgery for ganglia is "very low."3 The bottom line of this POEM is that patients with ganglia will be misled by being told not to worry as their painful lump is likely to do just as well by leaving it alone. And cash strapped local management will be only too happy to reinforce this unfortunate advice and tao.
This meta-analysis compared amiodarone with placebo and class Ic drugs for the cardioversion of recent-onset atrial fibrillation AF ; , defined as lasting less than seven days. BACKGROUND Despite the lack of trials that support its efficacy convincingly, amiodarone is widely used for conversion of recent-onset AF. METHODS We searched Medline and EMBASE databases, as well as the Cochrane Controlled Trials Register for randomized trials on recent-onset AF comparing amiodarone to placebo or class Ic drugs. Data were combined according to a fixed effect model. The primary end point was the rate of conversion at 24 h. study time-dependency of the drugs, efficacy at 1 to and at 24 h was analyzed. RESULTS We found six studies randomizing amiodarone versus placebo 595 patients ; and seven studies versus class Ic drugs 579 patients ; . There was no significant difference between amiodarone and placebo at 1 to but significant efficacy was found after 6 to 8 relative risk [RR] 1.23, p 0.022 ; and at 24 h 1.44, p 0.001 ; . Efficacy with amiodarone was inferior to class Ic drugs for up to 8 0.67, p 0.001 ; but no difference was seen at 24 h 0.95, p 0.50 ; . There were no major adverse effects. CONCLUSIONS Amiodarone is superior to placebo for cardioversion of AF, and even though the onset of conversion is delayed, its efficacy is similar at 24 h compared with class Ic drugs. These results favor amiodarone as a reasonable alternative for patients with recent AF in whom class Ic and other, more rapidly acting antiarrhythmic drugs cannot be used. As a result, the FDA strongly cautions consumers about purchasing drugs from any of these Web sites which may be involved in the distribution of counterfeit drugs and reiterates previous public warnings about buying prescription drugs online.
