Cigarette smoking can interact with some medicines. This is mainly due to polycyclic aromatic hydrocarbons in cigarette smoke that stimulate cytochrome P450 enzymes, particularly CYP1A2. Theophylline is metabolised via CYP1A2, and is eliminated considerably more rapidly in smokers compared with non-smokers. Higher doses are needed by patients who smoke and for heavy smokers the dose may need to be doubled. The dose of theophylline will need to be reduced if the patient stops smoking, or is admitted to hospital and unable to smoke. Serum theophylline levels should be monitored, and the patient counselled to seek help if they develop signs of theophylline toxicity such as palpitations and nausea. Cigarette smoking has the potential to interact with other medicines. The majority of interactions are not considered to be of clinical significance, but the potential for interaction should be borne in mind if a patient starts or stops smoking.
