The first, our distance learning training pack, in the form of a very detailed interactive CDRom, is being carefully developed under the watchful eye of Dympna Pearson, with the support of the Dept of General Practice at Leicester University and Dr Karen Slevin. True, it has taken a bit longer to develop than anticipated, but it will be the better for it, and promises to provide an excellent platform for all clinicians who want to increase their knowledge and practical skills in weight management. Scotti With the aim of tracking somatic stem cells upon transplantation or within gene therapy applications, we explored a cell labelling strategy based on MR reporter genes, allowing iron accumulation within cells, and we exploited the unique properties of late generation lentivirial vectors LV ; in transducing with high efficiency SSC both ex vivo and in vivo, and guaranteeing long term expression in their progeny. We tested two candidate MR reporter genes, human tyrosinase and ferritin. Mono- and bi-cistronic LV carrying the human tyrosinase hTyr ; cDNA downstream to the human phospoglycerokinase promoter were produced. hTyr LV and hTyr + GFP LV were used to transduce cells lines, hematopoietic and neural stem cells. Stable clones expressing H and L ferritin under doxicycline regulation were also analyzed. Transduced cells became detectable in T2 and T2 * weighted images, concomitantly with the appearance of a positive staining for melanin or iron within cellular bodies. Viability, proliferation and differentiation were preserved. Further, hTyr LV and hTyr + GFP LV were directly injected in the striatum of mice that underwent serial MR imaging examinations on a dedicated 7 Tesla and a human 3 Tesla MR scanners. The study was done to prospectively measure the echocardiographic, hemodynamic and clinical outcomes after partial left ventriculectomy PLV ; . BACKGROUND Although PLV can improve symptoms of advanced heart failure, immediate postoperative echocardiographic findings remain abnormal. METHODS Fifty-nine patients with cardiomyopathy and advanced heart failure underwent PLV and concomitant mitral valve surgery between May 1996 and December 1997. Thirty-nine percent were on inotropic therapy. All were New York Heart Association NYHA ; functional class III or IV. Mechanical circulatory support LVAD ; and transplant were provided for rescue therapy when hemodynamic compromise occurred. Patients were followed for a mean of 405 168 days, and clinical, echocardiographic and hemodynamic measures were obtained preoperatively, immediately postoperatively, and at 3 and 12 months prospectively. RESULTS Comparing preoperative and 12-month postoperative values in event-free survivors, we found: NYHA functional class improved from 3.6 to 2.1, p 0.0001; peak oxygen consumption increased from 10.8 to 16.0 ml kg min, p 0.0001; LV ejection fraction increased from 13 6.0% to 24 6.9%, p 0.0001; LV end diastolic diameter decreased from 8.2 1.03 to 6.2 0.64 cm, p 0.0001, and volume was reduced from 167 60 to 105 38 ml m2, p 0.02.
