TACE One to eight sessions of TACE were performed via the transfemoral arterial approach under local anesthesia at intervals of 4-12 wk in the TACE + LT group. Selective celiac and superior mesenteric angiography was performed to define the hepatic artery anatomy and to evaluate the portal venous system. The feeding artery to the lesion was catheterized as selectively as possible using a highly flexible coaxial catheter, and the chemotherapeutic agent lipiodol mixture was injected under fluoroscopic guidance. The mixture contained 20-50 mg of doxorubicin, 3-20 mL of lipiodol Lipiodol Ultrafluide; Guerbet, Aulnay-sousBis, France ; , and 3 mL of water-soluble contrast agent. Embolization was performed with gelatin pellets Gelfoam; Upjohn, Kalamazoo, Michigan ; thereafter in patients with liver function of Child-Pugh class A and tumor confined to single lobe of the liver. If viable tumor was still observed in CT, repeated TACE was performed. If lipiodol was compactly uptaken by all tumor nodules and any new lesion was not seen, follow-up CT scan was repeated every 3 mo and tarka. Although several therapeutic options have been advocated, transcatheter arterial chemoembolization TACE ; in particular has been widely performed in the treatment of HCC. Still, hepatic arteriography and portography are mandatory for evaluation of a ; the resectability and multiplicity of HCCs and b ; the hemodynamic status of the portal vein. Thereafter, TACE can be considered as the initial therapeutic modality. The possibility of nontarget organ complications during TACE eg, ischemic cholecystitis, splenic infarction, gastrointestinal mucosal lesions, pulmonary embolism and infarction, spinal cord injury, ischemic skin lesions ; should be taken seriously. A thorough understanding of the anatomic variants and hemodynamic features of the hepatic artery and portal vein is the first step in performing effective and safe TACE for HCC.

TACE Cleaves ProTGF In Vitro To assess the ability of TACE to directly cleave proTGF , lysates of RLEC stably expressing HA FLAG-tagged proTGF 34 ; were incubated in the absence or presence of native TACE. Consistent with previous observations 34 ; , anti-HA recognized prominent proTGF proteins of about 25 and 36 kDa in Western blots of reaction mixtures incubated in the absence of TACE Fig. 3A, left ; . In the presence of TACE, a prominent new species of about 20 kDa was detected, with corresponding decreases in larger proTGF species, especially the 25 kDa form. This 20 kDa product was readily detected after 15 min incubation with exogenous TACE, but was present at higher levels after 60 min. Its appearance was inhibited in the presence of IC-3. Minor amounts of the 20 kDa product were also detected when reactions were performed for 60 min in the absence of TACE, presumably due to endogenous TACE in RLEC 4, 5. Importantly, the novel, TACE-dependent 20 kDa-product was also recognized by anti-FLAG Fig. 3A ; , indicating that TACE cleaved only the N-terminal processing site of proTGF in vitro and not the C-terminal site. Although we occasionally observed modest increases in the levels of 16 kDa tail following incubation with TACE see Fig. 3A, 15 min time point ; , anti-HA did not detect mature TGF in these reactions, consistent with limited or absent cleavage of the C-terminal site. To confirm accurate cleavage by TACE, proTGF was partially purified by anti-FLAG affinity chromatography and incubated with recombinant human TACE ectodomain 36 .
